When addressing concerns such a loss of volume, wrinkles, and facial sagging, the approach that will yield the best result is to treat to full facial correction using multiple syringes of filler. During a cosmetic consultation, a cosmetic coordinator and a medical provider will determine a treatment package approach keeping in mind the best filler based on an individual’s skin, filler history, lifestyle and budget. Kybella, a drug approved by the FDA in April of 2015, is injected directly into the chin to dissolve fat in a “double chin”. How do we choose which filler to use?

At Advanced Dermatology, we prefer hyaluronic acid fillers such as, Restylane, Restylane Lyft, Juvederm, and RHA® because they look and feel natural when injected. The exception is when the amount of volume to be corrected is very large, as in very sunken cheeks. In this case, we turn to Sculptra, which builds volume by stimulating the patient to form his or her own collagen.

Hyaluronic acid fillers have many advantages. One is that hyaluronic acid is a natural component of our skin. It is the soft gel that surrounds and cushions the structural collagen molecules. Another is that treatment is reversible, meaning that if a problem arises, we can inject an enzyme that breaks down the injected hyaluronic acid, thus neutralizing the problem. A third advantage is that hyaluronic acid fillers have a long safety record.

Whether we inject hyaluronic acid, Sculptra, or both, our goal is to achieve complete correction with the initial treatments. This makes it easier to maintain the corrected volume. Maintenance treatments are needed every 6 to 12 months. During the maintenance period, some of the filler is reabsorbed by the body but some may remain in the corrected area. Over time the amount of filler needed for maintenance decreases, which reduces treatment time, reduces downtime, and saves money.

Before and after example

The woman in the before photograph has noticeable folds on both sides of her nose and at both corners of her mouth. To make these folds less noticeable we injected filler into the cheeks rather than into the folds themselves, as it was determined by cosmetic Dermatologists and Plastic Surgeons that facial wrinkles and folds are caused by pockets of volume loss. The improvement in both folds is apparent in the after photograph. In the after photo the woman looks more awake and rejuvenated. The shape of her face is more pleasing and she feels more attractive. In this case, restoring the volume of her cheeks lifted other areas of her face.
